This request form complies with the policies established by the NYBA for the 2010 American Legion Baseball Season. It should be only used for adding additional games that are not on the NYBA master schedule.

This form (AGO1 and AGO1 Page 2) must be faxed to and received by the Schedule Coordinator (Mike Pristow) no later than 14 days prior to the date of the requested additional game(s). Mike's fax number is 365-0454 - Note: This is also his home phone number - BE SURE TO CALL BEFORE FAXING. Charges for additional games will be held against the credit card of record for the team requesting the game. If full payment (cash or check) is directly received by the NYBA's Treasurer on or before 07-19-08, then the credit card will not be charged. After 07-19-08, all charges incurred for additional game(s) will be processed to the credit card.

If the two teams that involved in an Additional Game have chosen to share the costs of the umpires, ($125.00 per game for AAA and $125.00 per game for AA) it is up to the team making the request for the additional game to collect the shared costs from the opposing team. All applicable charges will be billed to the team that completes and signs this form.
